A Handful of Nuts a Day May Help Keep Hypertension Away

A handful of nuts is not a particularly dramatic dietary intervention. There is no complicated recipe, no supplement schedule and no need to reorganize an entire diet around it. Yet a new analysis suggests that this modest habit may be associated with a meaningful difference in the risk of developing high blood pressure.

The British Journal of Nutrition published a paper that reviewed eight prospective studies involving 143,380 adults, 20,665 of whom developed hypertension. When they compared people with the highest nut consumption with those eating the least, the higher-intake group had a 16 percent lower relative risk of hypertension.

The more interesting finding emerged when the researchers examined how risk changed as nut consumption increased. For every additional serving of approximately 28 grams — about one ounce, or a small handful — per day, hypertension risk was about 20 percent lower in the statistical analysis. The relationship did not appear to continue indefinitely, however. The curve began to flatten at around 25 to 30 grams a day, with the model suggesting the greatest reduction — approximately 26 percent — around 30 to 35 grams.  In practical terms, that is not a large quantity: roughly the amount of nuts that fits in the palm of a hand.

There are plausible reasons nuts might be helpful for blood pressure. They provide unsaturated fats, fiber and minerals including magnesium and potassium, while also containing a range of plant compounds with potential cardiovascular effects. Previous randomized trials have generally pointed in the same direction, although the reductions in blood pressure itself have tended to be modest. An earlier dose-response analysis of 31 randomized trials, for example, found that increasing nut intake by 20 grams a day lowered systolic pressure by about half a millimeter of mercury on average.

That distinction is important. The new study did not show that eating nuts caused a 20 or 26 percent reduction in hypertension. It combined observational studies in which researchers followed people over time and compared their reported diets with their subsequent risk of developing the condition.

People who regularly eat nuts may differ from those who do not in ways that are difficult to completely separate statistically. They may have different overall diets, exercise habits, body weights or other health behaviors. Dietary intake was also largely based on participants' reports, often measured only at the beginning of a study. The authors therefore acknowledge that residual confounding and measurement error remain possible.

There was another surprisingly important piece of information the researchers did not have: whether the nuts were salted. That matters when the outcome being studied is blood pressure. Nuts themselves may fit comfortably into a heart-healthy diet; covering them generously with sodium could complicate the equation. The analysis also could not determine whether walnuts, almonds, pistachios, pecans or other individual nuts differed in their association with hypertension.

Still, the findings add to a much larger body of evidence suggesting that nuts deserve a regular place in the diet. What is particularly useful about this study is the dose-response analysis. Nutrition research often leaves consumers with vague instructions to eat “more” of something. Here, more did not necessarily mean better forever. Much of the association appeared around a remarkably ordinary amount.

About an ounce once a day.  It is unlikely to transform blood pressure on its own, and it certainly isn't a substitute for treating hypertension when treatment is needed. But as dietary changes go, it may be one of the easier ones to put into practice.
